Disparities in the care of people with advanced heart disease and outcomes for adult transplant recipients have persisted for decades in Austin and throughout the nation. This forum will explore how team-based care impacts patient experience, quality of care and outcomes as well as whether pharmacogenetics influence practice. Participants will also gain insight on the role of health care policies and regulations in the provision of equitable care.
