Hello, you are being invited to participate in a research study investigating what influences readiness to transition from pediatric to adult healthcare. This is a one-time online survey that will take approximately 20-45 minutes to complete. You will be asked a series of questions to determine what factors influence transition readiness.

To be eligible you must

• Between the ages of 18-25,
• Be an English speaker,
• Able to answer survey questions independently.
• Must also be diagnosed with a chronic condition, examples include but are not limited to, Inflammatory bowel disease (IBD), Congenital Heart Condition or other heart condition, Sickle Cell Disease, Diabetes, or other Endocrine Disorder, Muscular Dystrophy, Cystic Fibrosis, Epilepsy, Asthma etc.

To thank you for your time in completing this survey, you will be entered into a drawing for a chance to win one of two $100.00 electronic gift cards at the end of this survey.
