The objective of this study, "Bridging the Gap: Chronic Disease Self-Management Transitions," is to determine the information that patients need and want during hospitalization to help them plan for discharge. Self-management of a chronic disease, such as diabetes, autoimmune disease, or cancer, is the focus of this study. Your participation in the survey will contribute to a better understanding of how healthcare providers can meet the information needs of patients during hospitalization. 

To participate you must live in the United States, be at least 18 years old, currently or previously been diagnosed with a chronic disease, and have been hospitalized since your chronic disease diagnosis. The hospitalization does not have to be directly related to your chronic disease. 

You will be asked to recall how information was shared with you during your hospitalization, if you had the information you needed when you got home, and how you think information sharing during hospitalization can be improved.
